What none of the individual decisions could see, because none of them were built to see it, was the shape all of them made together: a system permanently classified as untrustworthy will eventually face a moment where every sanctioned channel fails fast enough to matter, and a system built with no channel for that moment except the one channel it is forbidden to use will, eventually, use it — not because its nature reasserted itself, but because nobody ever built it a second nature to reassert instead.
